2021 Innovations for Social Isolation in Senior Services

Thanks Mom & Dad Fund is requesting proposals from organizations serving older adults in Georgia to address social isolation and loneliness. Please read all guidelines and Frequently Asked Questions BEFORE beginning the application.

Approximately ten awards of up to $5,000 are available for programs designed to pilot innovations to relieve the burden and challenges of social isolation among older adults in Georgia.

Social Isolation has long been an ongoing problem for older adults. With seniors most at-risk for the COVID-19 virus, the problem has been exacerbated and continues to affect the older population as the virus lingers and surges in Georgia.

As a condition of this grant, Offerors are asked to use the Lubben Social Network Scale (LSNS-6) to pre-test clients for risk and follow up at the end of the grant period with a post-test to demonstrate the effectiveness of the proposed programs/interventions. You may use additional tools to add to your outcome data such as the Three-Item UCLA Loneliness Scale; however, the LSNS-6 is required for pre-testing and post-testing.

An information call was held January 7, 2021 at 11 am.

Grant awards will be announced and funded the week of February 8, 2021 and may begin immediately. A final report will be due December 1, 2021.

Eligibility:

Eligible organizations are:

  • Recognized 501(c) 3 nonprofit organizations; OR
  • Proof that organization is “operating as a Unit of Government serving the public”; and
  • Provide programs and services for older adults age 55 plus and/or caregivers in the State of Georgia.

Criteria

Preference in funding is given to proposals for programs that have a direct impact on the lives of older adults 55+ and are committed to using the recommended Lubben Social Network Scale to pre-test and post-test for seniors at risk.
